Assessment Support Request Form

Submitted requests forms will go to the Office of Institutional Research and Assessment. Please allow a minimum of two weeks prior to your requested deadline.

  • For Academic Affairs please provide the assessment level. This will either be the major or program (major + concentration). For Student Affairs please provide your department. For all others please type in N/A.

  • Please attach any documents you would like IR&A to review before the meeting:

  • If you need to upload multiple files, please place all of the files in a folder and zip the folder before uploading.
