Enrollment Verification Requests

To request enrollment verification, current/recent students should visit Becker InSite to submit a request directly. Students without an InSite account, please click here.

Requests should be submitted in writing (or via Becker InSite) and must include the following information:

  • First and last name while attending and current last name (if applicable); any other names used
  • Student ID number or Social Security number
  • Date of birth
  • Current home address
  • Current telephone number
  • Campus location(s) attended
  • Dates of attendance
  • Contact name and complete address(es) of where the verification is to be mailed; fax number if preferred
  • Signature

Additional information to be included on verification (honors, CGPA). NOTE: If you are requesting additional information, we must have your request in writing with your signature, or via InSite, or the additional information will not be provided.

We will accept and process outside verification forms, such as student loans, insurance, employment background check, etc. If applicable, please complete your section of the form and sign before turning the form in to the Office of the Registrar.
